December 2026 State License Renewals: 12 States and a Checklist

Sandy Carter

Facility licenses in 14 programs across 12 states expire on December 31, 2026. With fewer than 60 days left, you're well past the ideal 90-day head start. These states include: Alabama, Arkansas, Idaho, Illinois, Iowa, Louisiana, Maine, Mississippi, New Hampshire, New Mexico, North Carolina, and South Dakota. Miss the deadline, and your pharmacy, wholesaler, manufacturer, 3PL, or outsourcing facility can't legally operate in that state.

Know Your State's Rules

Every agency runs renewal its own way, and the details that trip people up rarely appear in the general instructions. Methods, windows, fees, and required attachments also change between cycles, often without notice.

Check these for each license:

  • ‍Order of renewal: Some states require an individual license (designated representative or pharmacist-in-charge) to be current before the facility permit. In Alabama, renew the designated representative's registration first, or the board will hold or reject the facility renewal.‍
  • Renewal method: If you renew by mail-in paper application, check for the most up-to-date renewal form. For online portals, make sure you have the proper URL and have created an account ahead of time.‍
  • Fees: Fees vary by permit type, and several states charge a separate fee if you handle controlled substances. Paying short counts as not paying.‍
  • Renewal window: Windows open on a set date and not before. Make sure to verify when the renewal period opens for each state.‍
  • Overlapping cycles: Certain licenses have either annual or biennial expiration dates, and both may be due this year. In Alabama, manufacturer, wholesaler, 3PL, and pharmacy permits all expire 12/31/2026.

Confirm Every Expiration Date

Take every expiration date from the license or the board portal, not last year's paperwork. Renewal dates can change from the previous year.

Different license types can expire on different dates, even in the same state. Wholesaler permits, DEA registrations, and controlled substance licenses rarely expire together, so track the expiration date for each one.

Three programs need particular care:

  • ‍New Hampshire mail-order pharmacy: only even-numbered licenses expire 12/31/2026.‍
  • New Mexico pharmacy, wholesale, and manufacturer: the two-year cycle runs from the issue year.‍
  • Iowa 3PL: 3PL licenses renew March 31. Pharmacy, wholesale, and outsourcing licenses still expire 12/31/2026.

Your Renewal Checklist

Start with the slow steps. A stale inspection report stalls more renewals than anything else and takes the longest to fix.

By November 13

  • Confirm every expiration date and set a December 1 reminder.
  • Call each board. Ask what's changed this cycle and what your facility type needs to submit.
  • Check each inspection report's date against the board's window, usually one to two years. If it's out of range, or you've changed layout, storage, security, or services since the last inspection, schedule a new one now.

By November 25

  • Renew any individual license first, that is required for the facility license renewal, such as a designated representative or PIC license.
  • Gather the documents the board requires. Expect the license number, PIC name and license number, inspection report, DEA or controlled substance number, ownership details, and fee.
  • Confirm the exact fee for each permit.
  • List every disciplinary action since the last renewal, with date, outcome, and corrective steps, even ones you've already reported.

Before December 1

  • File online the day each window opens. If the state only takes paper, mail it now.
  • Keep every confirmation number and receipt.

December 1 to 31

  • Chase anything not yet approved. Filing on time doesn't mean the board will process it in time.

Don't Let It Lapse

A lapsed license means you can't ship, dispense, or distribute until you reinstate it, no matter how many years you've renewed on time. The cause is almost never a surprise. Catch problems now, while there's still time to fix them.

Track Every Renewal Date in One Place

A missed renewal date means you cannot ship, dispense, or distribute, and holding licenses in several states makes dates easy to lose.

LighthouseAI Management centralizes all of your facility licenses in one place and sends 30, 60, and 90-day reminders before each license expires, so your team starts every renewal with time to spare.
